If you have been recently diagnosed with clinically low-risk prostate cancer (Gleason score of 3+4 or less than 7), you may be eligible for the Oncotype DX Genomic Prostate Score (GPS) test, which is designed to help you and your doctor make a more confident decision about your treatment plan. The majority of treatable/treated cancers are of Gleason scores 5 - 7 and are detected due to biopsy after abnormal digital rectal exam or prostate specific antigen evaluation. A Gleason score of 7 can be 3+4= 7, where the majority is pattern 3, or it can be 4+3=7 which pattern 4 dominates resulting in a more advanced cancer. Because the two most common types of cancer cells are identified in the prostate, the Gleason score is a combination of these two cell types. For example, if the most common cell type is a 3 (on the 1 to 5 scale) and the second most common type is a 4, then the Gleason score is reported as a 7 or sometimes as a “3+4.” The Gleason score is currently the most useful tool for predicting the behavior of a prostate cancer and determining the best treatment options. It is used by your doctor in combination with the cancer stage and other variables to guide treatment decision making.
Theoretically, Gleason scores range from 2-10. However, since Dr. Gleason’s original classification, pathologists almost never assign scores 2-5, and Gleason scores assigned will range from 6 to 10, with 6 being the lowest grade cancer. The Gleason Score ranges from 1-5 and describes how much the cancer from a biopsy looks like healthy tissue (lower score) or abnormal tissue (higher score). Most cancers score a grade of 3 or higher. Since prostate tumors are often made up of cancerous cells that have different grades, two grades are assigned for each patient. The cancer is likely to grow slowly: Gleason score 7 (or 4 + 3 = 7) Grade Group 3: The cells A Gleason score of 8 or higher, accompanied by a PSA level of higher than 20 ng/ml and a more advanced tumor stage, signifies a high risk of advancing cancer.
